Before You Apply
Requirements
Prepare these requirements before visiting the office.

Laboratory request form
Where to secure: Fernandino Dialysis Center (City Civic Center, Brgy. San Isidro, City of San Fernando, Pampanga)

Negative antigen test result, if needed
Where to secure: CSFP Central Laboratory

Medical certificate or medical abstract
Where to secure: Specialist or attending physician

Prescription for medicines, if any
Where to secure: Specialist or attending physician

Applies only: Submit one accepted identification document
Photocopy of any government-issued ID (preferably showing name, address, birthday, signature, and picture)

Steps
How to apply

Submit your requirements at the Rural Health Unit.

Allow Rural Health Unit staff to take your vital signs and conduct an initial physical examination before you are referred to the physician for the medical certificate.

Wait for the Rural Health Unit to inform you that your document is ready for claiming.
- 4
Claim your document at the Rural Health Unit and sign the receiving logbook.
