All dialysis patients of the Fernandino Dialysis Center requesting medical clearance via medical certificate.

Laboratory request form
Saan kukunin: Fernandino Dialysis Center (City Civic Center, Brgy. San Isidro, City of San Fernando, Pampanga)

Negative antigen test result, if needed
Saan kukunin: CSFP Central Laboratory

Medical certificate or medical abstract
Saan kukunin: Specialist or attending physician

Prescription for medicines, if any

Naaangkop lamang: Submit one accepted identification document
Photocopy of any government-issued ID (preferably showing name, address, birthday, signature, and picture)

Paano mag-apply

Submit your requirements at the Rural Health Unit.

Allow Rural Health Unit staff to take your vital signs and conduct an initial physical examination before you are referred to the physician for the medical certificate.

Wait for the Rural Health Unit to inform you that your document is ready for claiming.

Claim your document at the Rural Health Unit and sign the receiving logbook.
