Before You Apply
Requirements
Prepare these requirements before visiting the office.

Applies only: For an applicant with an apparent disability applying for a new PWD ID
Photocopy of any government-issued ID (preferably showing name, address, birthday, signature, and picture)
Where to secure: Requesting client or issuing agency

Applies only: For an applicant with an apparent disability applying for a new PWD ID
Previous medical records and/or prescriptions from a medical specialist or private attending physician, if any

Applies only: For an applicant with a non-apparent disability applying for a new PWD ID
Photocopy of any government-issued ID (preferably showing name, address, birthday, signature, and picture)

Medical certificate and/or latest prescription from a medical specialist or private attending physician
Where to secure: Referral to a medical specialist or private attending physician

Photocopy of the previous Persons with Disability ID

Steps
How to apply

Submit your requirements at the Rural Health Unit.
- 2
Allow Rural Health Unit staff to take your vital signs and conduct an initial physical examination before you are referred to the physician for the medical certificate.

Wait for the Rural Health Unit to inform you that your document is ready for claiming.
- 4
Claim your document at the Rural Health Unit and sign the receiving logbook.
