A public health initiative designed to certify an individual's disability based on the supporting documents and records the client submits.

Ihanda ang mga kinakailangang ito bago bumisita sa tanggapan.

Naaangkop lamang: For an applicant with an apparent disability applying for a new PWD ID
Photocopy of any government-issued ID (preferably showing name, address, birthday, signature, and picture)
Saan kukunin: Requesting client or issuing agency

Previous medical records and/or prescriptions from a medical specialist or private attending physician, if any

Photocopy of any government-issued ID (preferably showing name, address, birthday, signature, and picture)

Medical certificate and/or latest prescription from a medical specialist or private attending physician
Saan kukunin: Referral to a medical specialist or private attending physician

Photocopy of the previous Persons with Disability ID

Mga Hakbang
Paano mag-apply

Submit your requirements at the Rural Health Unit.

Allow Rural Health Unit staff to take your vital signs and conduct an initial physical examination before you are referred to the physician for the medical certificate.

Wait for the Rural Health Unit to inform you that your document is ready for claiming.
- 4
Claim your document at the Rural Health Unit and sign the receiving logbook.
