Steps
How to apply

Submit the Request Form and applicable authority and identification documents at CCRO Window 45.
- 2
Receive the order of payment, pay PHP 50 at City Treasurer's Office Window 37, and return the official receipt.
- 3
Wait while CCRO verifies the local record and prepares the certified transcript or photocopy.
- 4
Receive the City CCRO-certified local copy.

- The output is a City CCRO/local civil-registry certified copy or transcription, not a PSA Security Paper certificate.
- The Charter permits a scanned authorization or SPA in applicable cases; this does not establish email or instant messaging as the application channel.
