CCRO receives eligible administrative petitions under RA 9048 and RA 10172 for clerical errors, change of first name, change of sex, or correction of the day or month of birth without a judicial order.

Requirements
Prepare these requirements before visiting the office.

Erroneous PSA civil-registry record and petitioner identification or certification (3 photocopies each)

Steps
How to apply

Submit the applicable petition and complete supporting documents to CCRO.

Pay the applicable petition fee at the City Treasurer's Office and return the official receipt.

Complete any required posting or publication steps and await CCRO processing and endorsement.
- 4
Follow up after the separate PSA Legal Services review period.

Important notes
- The Charter total is 10 days, 1 hour, and 10 minutes. It separately estimates 3–4 months depending on PSA Legal Services processing; these periods are not added together.
- The governing references are RA 9048 and RA 10172. PSA national annotation and certificate issuance remain separate from CCRO processing.
