MAYOR’S PERMIT FOR BUSINESS (RENEWAL)
The Business License and Permit Division under the City Administrator's Office is directly responsible for the issuance of a Mayor's/Business Permit for regulating the operation of businesses within the territorial jurisdiction of the City of San Fernando, Pampanga. Any person doing business in the City of San Fernando, Pampanga can avail of this service.

Proof of annual gross receipts (original for presentation; 1 photocopy), which may be audited financial statements, unaudited financial statements for those not required to file AFS with the BIR, a sworn declaration of gross sales or receipts, or Income Tax Returns

License to work for entertainers/models and Cervical Urethral Smear results from the Social Hygiene Clinic for GROs (1 original for presentation; 1 photocopy)

Applies only: For water refilling stations, water works systems, bulk water, water vending machines, and ice manufacturers
Applicable microbiological test results for Total Coliform, E. coli, and Heterotrophic Plate Count; Certificate of Potability; and applicable physical/chemical test results following the mandatory parameters of PNSDW 2017. Testing frequency and raw/product water requirements depend on whether the source is a deep well or Water District

Steps
How to apply
- 1
Accomplish the application form and submit all necessary documents at the Business License and Permit Division, Windows 12-16. If the application is on the negative list, comply first with the concerned regulatory office or offices before assessment.

Pay the corresponding taxes and fees at the City Treasurer's Office, Windows 6 or 10. Receive the Official Receipt, Community Tax Certificate, and Claim Stub, then return on the date indicated in the stub.

Claim the Mayor's Permit at the Business License and Permit Division, Windows 61 or 62, complete the Claim Stub, and drop it in the Suggestion Box.
