   Saan kukunin: Business License and Permit Division, Windows 11-16

2. 2.1

   Naaangkop lamang: For declaration of gross sales

   Proof of annual gross receipts (original for presentation; 1 photocopy), which may be audited financial statements, unaudited financial statements for those not required to file AFS with the BIR, a sworn declaration of gross sales or receipts, or Income Tax Returns

   Saan kukunin: Bureau of Internal Revenue; Business owner; Notarial Services; Company accountant

   Naaangkop lamang: For banks

   Notarized Joint Statement of Annual Income signed by a designated officer of the head office and by the branch manager (1 original)

   Saan kukunin: Company head office and branch manager; Notarial Services

   Fire Safety Inspection Certificate

   Saan kukunin: Bureau of Fire Protection

   Sanitary Permit, Environmental Management Permit, clearances from OCBO and the Barangay, and Community Tax Certificate

   Saan kukunin: Facilitated by the Business License and Permit Division

   Naaangkop lamang: If the applicant is a representative of a single proprietorship

   Authorization Letter and photocopies of any government-issued ID of the owner and representative, with specimen signatures

   Saan kukunin: Owner

   Naaangkop lamang: If the applicant represents a corporation, partnership, cooperative, or association

   Secretary's Certificate or Board Resolution and photocopy of any government-issued ID of the representative, with specimen signature

   Saan kukunin: Corporation, partnership, cooperative, or association

   Naaangkop lamang: For amusement entities

   Grant of authority or franchise from Congress, PAGCOR, or the Sangguniang Panlungsod (1 photocopy; original for presentation)

   Saan kukunin: Congress, PAGCOR, or Sangguniang Panlungsod

   Naaangkop lamang: For pawnshops, foreign exchange dealers, money changers, and remittance agents that were new businesses in the previous year

   BSP proof of registration, either a received application form or Certificate of Registration (1 photocopy)

   Saan kukunin: Bangko Sentral ng Pilipinas

10. D

    Naaangkop lamang: For night clubs and entertainment establishments

    License to work for entertainers/models and Cervical Urethral Smear results from the Social Hygiene Clinic for GROs (1 original for presentation; 1 photocopy)

    Saan kukunin: Rural Health Unit II, Barangay Health Center-Sindalan

11. E

    Naaangkop lamang: For water refilling stations, water works systems, bulk water, water vending machines, and ice manufacturers

    Applicable microbiological test results for Total Coliform, E. coli, and Heterotrophic Plate Count; Certificate of Potability; and applicable physical/chemical test results following the mandatory parameters of PNSDW 2017. Testing frequency and raw/product water requirements depend on whether the source is a deep well or Water District

    Saan kukunin: Water Laboratory

    Naaangkop lamang: For food establishments, restaurants, fast-food businesses, canteens, eateries, and caterers

    Letter designating a Food Safety Compliance Officer with proof of prescribed FSCO training, or a commitment to attend FSCO training

    Saan kukunin: Department of Health or City Health Office, ground floor

    Naaangkop lamang: For water laboratories

    Valid DOH Certificate of Accreditation, current NRL Certificate of Proficiency, and NRL License Certificate for the water sampler

    Saan kukunin: Department of Health; National Reference Laboratory
