Issuance of Solo Parent Identification Card for Applicants' Renewal
A Solo Parent ID is a government-issued identification card for solo or single parents. The CSWDO renews the card for a registered solo parent, supporting access to certain government discounts and benefits for residents of the City of San Fernando.

Photocopy of Voter's ID, Voter's Certification, or another accepted valid ID with specimen signature. Accepted IDs include ePhilID, Philippine Passport, SSS ID or UMID Card, GSIS ID or UMID Card, Driver's License, PRC ID, OWWA ID, Senior Citizen ID, PWD ID, NBI Clearance, PhilHealth ID (digitized PVC), Integrated Bar of the Philippines ID, School ID (for students), TIN ID, Postal ID, Barangay Certification, GSIS E-CARD, Certification from the National Council for the Welfare of Disabled Persons, Company ID issued by a private entity, Police Clearance, Cedula or Community Tax Certificate, Form 137 (for students), Transcript of Records, Marriage Contract, or Birth Certificate.

Steps
How to apply

Surrender the old Solo Parent ID, undergo the initial validation interview at the CSWDO, and receive the checklist of requirements for renewal.

Submit the completed requirements and the Solo Parent Application Form to the Solo Parent Focal Person at the CSWDO.
- 3
Wait for the approval of the application. Once approved, sign in at the logbook for ID release and claim the renewed Solo Parent ID from the Solo Parent Desk Officer/Focal Person on the given date.
