Issuance of Person with Disability Identification Card and Purchase Booklets for New Applicants
The PDAO issues a Person with Disability (PWD) Identification Card and purchase booklets to a new applicant. The PWD ID supports access to the 20% discount and value-added-tax exemption on certain goods and services for residents of the City of San Fernando.

Barangay Certificate of Residency (1 original, 1 photocopy)

Applies only: Submit one accepted identification document
Photocopy of Voter's ID, Voter's Certification, or another accepted valid ID with specimen signature. Accepted IDs include ePhilID, Philippine Passport, SSS ID or UMID Card, GSIS ID or UMID Card, Driver's License, PRC ID, OWWA ID, Senior Citizen ID, PWD ID, NBI Clearance, PhilHealth ID (digitized PVC), Integrated Bar of the Philippines ID, School ID (for students), TIN ID, Postal ID, Barangay Certification, GSIS E-CARD, Certification from the National Council for the Welfare of Disabled Persons, Company ID issued by a private entity, Police Clearance, Cedula or Community Tax Certificate, Form 137 (for students), Transcript of Records, Marriage Contract, or Birth Certificate.

Medical Certificate (1 original, 1 photocopy) indicating the disability
Where to secure: Referral to City Medical Doctor, Rural Health Physician, and/or a specialist on the disability
Steps
How to apply

Prepare the required documents and complete the PWD ID application form at the Persons with Disabilities Affairs Office (PDAO).

Wait while PDAO staff review the submitted documents, and sign in at the CSWDO logbook. Answer any questions for clarification.

Wait for the approval and verification of the application. On the given date, sign in at the CSWDO logbook and claim the PWD ID from the CSWDO.

Important notes
- National guidance distinguishes documentary requirements for apparent versus non-apparent disability (a Certificate of Disability is generally not required for apparent disability); the local Charter checklist does not draw this distinction explicitly. This publication preserves the Charter checklist without a City-issued update.
