Issuance of Certificate of Transfer (PWD ID Records)
The PDAO issues a Certificate of Transfer certifying that a registered Person with Disability (PWD) in the City of San Fernando's registry is transferring residence to another city or municipality, to support endorsement of records and continuity of benefits.

Steps
How to apply

Submit the required documents at the Persons with Disabilities Affairs Office (PDAO).
