Acceptance of Certificate of Transfer (PWD ID Records)

The PDAO accepts a Certificate of Transfer certifying that a registered Person with Disability (PWD) from another city or municipality is transferring residence to the City of San Fernando, and requests inclusion in the City's official PWD Registry to support continuity of benefits and services.

Requirements

Prepare these requirements before visiting the office.

    Original Certificate of Transfer from the previous city or municipality

    Where to secure: PDAO of the previous local government unit

    Original PWD ID issued by the previous local government unit (for verification)

    Letter stating the reason for transfer and the date of transfer

    Barangay Clearance indicating residency in the City of San Fernando
