How to apply

Submit the accomplished Death Certificate form, correctly signed by the proper parties, for review.

Wait while staff review the form for correctness, completeness, and required signatures, and route it for the required certification and signing.

Claim the reviewed form and proceed to the City Treasurer's Office (Window 37) for payment and the City Civil Registry Office (Window 46) for registration.
