Issuance of Sanitary Permit (Renewal – Water Refilling Stations, Water Works System, and Bulk Water)

City Health Office (CHO)

The City Health Office – Environmental Health and Sanitation Division (EHSD) is responsible for the issuance of Sanitary Permits. All business establishments are required to secure a Sanitary Permit upon application for a Business Permit to ensure that the establishment complies with the laws and/or ordinances pertaining to health and sanitation in the City.

₱200.00 to ₱1,200.00, based on the fee schedule in Article 48 of the City's Enacting Ordinance and the 2017 Revised Revenue and Market Code.

Requirements

Prepare these requirements before visiting the office.

    Application Form for Business Permit

    Where to secure: Business One Stop Shop (City Hall)

    Microbiological Test: Total Coliform, E. Coli, and Heterotrophic Plate Count, with Certificate of Potability. If the water source is a deep well: product water tested monthly, raw water tested every six (6) months. If the water source is a water district: product water tested monthly.

    Where to secure: DOH-Accredited Water Laboratory / City Health Office

    Physical/Chemical Test following the mandatory parameters of the Philippine National Standards for Drinking Water (PNSDW 2017), with Certificate of Potability. If the water source is a deep well: raw and product water tested every six (6) months. If the water source is a water district: product water tested every six (6) months.

    Submit the required documents to the City Health Office – Environmental Health and Sanitation Division (EHSD) and wait while staff verify them.

    Wait while EHSD staff encode your information into the sanitary permitting system and print the Sanitary Permit.
