Issuance of Sanitary Permit (Renewal Business – Night Clubs/Entertainment Establishments)
The City Health Office – Environmental Health and Sanitation Division (EHSD) is responsible for the issuance of Sanitary Permits. All business establishments are required to secure a Sanitary Permit upon application for a Business Permit to ensure that the establishment complies with the laws and/or ordinances pertaining to health and sanitation in the City.

Result of the Cervical/Urethral Smear of Entertainment Workers
Where to secure: Social Hygiene Clinic (located at the City Civic Center, Barangay San Isidro, CSFP)

How to apply

Submit the required documents to the City Health Office – Environmental Health and Sanitation Division (EHSD) and wait while staff verify them.

Wait while EHSD staff encode your information into the sanitary permitting system and print the Sanitary Permit.

Wait for review and approval of the Sanitary Permit.

Receive the Sanitary Permit.
