Issuance of Sanitary Permit (New Business – Water Refilling Stations, Water Works System, and Bulk Water)
The City Health Office – Environmental Health and Sanitation Division (EHSD) is responsible for the issuance of Sanitary Permits. All business establishments are required to secure a Sanitary Permit upon application for a Business Permit to ensure that the establishment complies with the laws and/or ordinances pertaining to health and sanitation in the City.

Requirements
Prepare these requirements before visiting the office.

Application Form for Business Permit. For Water Refilling Stations, Water Works System, and Bulk Water: 5 original copies of the requirements are required (1 for the CHO-EHSD file, 4 for the DOH file).

Certificate of Certified Water Operator Training Course or Letter of Commitment

Sanitary Plan
Where to secure: Licensed Sanitary Engineer

Physical/Chemical Test following the mandatory parameters of the Philippine National Standards for Drinking Water (PNSDW 2017), with Certificate of Potability. If the water source is a deep well: raw and product water tested every six (6) months. If the water source is a water district: product water tested every six (6) months.

Sanitary Engineer's Report
Where to secure: Licensed Sanitary Engineer

Microbiological Test: Total Coliform, E. Coli, and Heterotrophic Plate Count, with Certificate of Potability. If the water source is a deep well: product water tested monthly, raw water tested every six (6) months. If the water source is a water district: product water tested monthly.

How to apply

Submit the required documents to the City Health Office – Environmental Health and Sanitation Division (EHSD) and wait while staff verify them.

Wait while EHSD staff encode your information into the sanitary permitting system and print the Sanitary Permit.

Wait for review and approval of the Sanitary Permit.

Receive the Sanitary Permit.
