Issuance of Sanitary Permit (New Business – Food Establishments, Restaurant, Fastfood, Canteen, Eatery, and Caterers)

City Health Office (CHO)

The City Health Office – Environmental Health and Sanitation Division (EHSD) is responsible for the issuance of Sanitary Permits. All business establishments are required to secure a Sanitary Permit upon application for a Business Permit to ensure that the establishment complies with the laws and/or ordinances pertaining to health and sanitation in the City.

₱200.00 to ₱1,200.00, based on the fee schedule in Article 48 of the City's Enacting Ordinance and the 2017 Revised Revenue and Market Code.

Requirements

Prepare these requirements before visiting the office.

    Application Form for Business Permit

    Where to secure: Business One Stop Shop (City Hall)

    Letter of designation of Food Safety Compliance Officer (FSCO)

    Where to secure: Food Business Operator (FBO)

  3. 3

    Proof that the FSCO passed the prescribed training course

    Where to secure: DOH-recognized third-party Training Course Provider

  4. 4

    Applies only: Required if proof of a completed training course is not yet available

    Letter of commitment to attend the prescribed FSCO training course

    Where to secure: Food Business Operator (FBO)

How to apply

    Submit the required documents to the City Health Office – Environmental Health and Sanitation Division (EHSD) and wait while staff verify them.

    Wait while EHSD staff encode your information into the sanitary permitting system and print the Sanitary Permit.

  3. 3

    Wait for review and approval of the Sanitary Permit.

  4. 4

    Receive the Sanitary Permit.
