Before You Apply

Requirements

Prepare these requirements before visiting the office.

    Laboratory results: chest X-ray (6 months' validity, 1 original copy), complete blood count with platelet (3 months' validity, 1 original copy), urinalysis (taken on or the day before consultation, 1 original copy), 12-lead ECG (3 months' validity, 1 original copy), and fecalysis (taken on or the day before consultation, 1 original copy)

How to apply

    Submit your requirements at the Rural Health Unit.

    Allow Rural Health Unit staff to take your vital signs and conduct an initial physical examination before you are referred to the physician for the medical certificate.

    Wait for the Rural Health Unit to inform you that your document is ready for claiming.

  4. 4

    Claim your document at the Rural Health Unit and sign the receiving logbook.
