Before You Apply

Requirements

Prepare these requirements before visiting the office.

    Original copy of the Treatment Booklet (Form 5) issued upon enrollment/admission

    Where to secure: Client's own booklet

    Sputum follow-up result:

    Negative sputum follow-up after 2 weeks from initiation of treatment, for bacteriologically-confirmed TB patients

    Where to secure: Rural Health Unit

  4. 2.2

    Negative sputum follow-up after 1 week from initiation of treatment, for clinically-diagnosed TB patients

    Where to secure: Rural Health Unit

  5. 3

    Applies only: Submit one accepted identification document

    Photocopy of any government-issued ID (preferably showing name, address, birthday, signature, and picture). Accepted IDs include Voter's ID, National ID, Unified Multi-Purpose ID (UMID), Postal ID, Tax Identification Number (TIN) ID, PRC ID, Driver's License, and Senior Citizen ID.

Steps

How to apply

  1. 1

    Submit your requirements and wait while staff review them for completeness and validity.

  2. 2

    Allow the health worker to take your vital signs and conduct an initial physical examination before you are referred to the physician for signing of the medical certificate.

  3. 3

    Wait for confirmation that your documents are available for claiming.

  4. 4

    Claim the requested documents at the Rural Health Unit and sign the receiving logbook.
