Steps
How to apply

Submit your requirements and wait while staff review and validate them.

Allow the health worker to check your vital signs and conduct an initial physical examination before referral to the physician for signing of the medical certificate.

Settle payment at the City Treasurer's Office.

Wait for confirmation that your documents are available for claiming.
- 5
Claim your requested document at the Rural Health Unit and sign the receiving logbook.
