Issuance of Health Certificate
The City Health Office – Environmental Health and Sanitation Division (EHSD) is responsible for the issuance of health certificates. Any person or individual can request a health certificate for employment.

Before You Apply
Requirements
Prepare these requirements before visiting the office.

Current or annual medical results for all workers/employees, consisting of:

Chest x-ray, current year (1 original)
Where to secure: Any DOH-accredited laboratory within the City of San Fernando, Pampanga
- 1.2
Urine test, at most one month upon application (1 original)
Where to secure: Any DOH-accredited laboratory within the City of San Fernando, Pampanga

Stool test, at most one month upon application (1 original)
Where to secure: Any DOH-accredited laboratory within the City of San Fernando, Pampanga

Steps
How to apply
- 1
Submit your diagnostic/laboratory results for validation, then pay the health certificate fee.

Present your diagnostic/laboratory results at the City Treasurer's Office, Window 3, for normal findings, or at the City Medical Unit for a medical check-up if there are abnormal findings.

Present the official receipt of payment.

Wait while your health certificate is prepared and processed.
- 5
Claim the health certificate and sign the logbook.
