Issuance of Death Certificate (Out-of-Facility Death, Medically Unattended)

City Health Office (CHO)

The City Health Office is responsible for reviewing death certificates. It examines the cause of death, issues the certificate, and directs the registration of the death at the City Civil Registry within the reglementary period of 30 days. The spouse or nearest relative of a person who died without medical attendance must report the deceased within 48 hours.

Steps

How to apply

  1. 1

    Accomplish the death certificate form with the deceased's personal data and information.

  2. 2

    Wait while staff record the deceased's details.

  3. 3

    Provide the details needed, including the cause of death to be signed by the Rural Health Physician, and the embalmer's signature at the back of the death certificate.

  4. 4

    Submit the additional requirements needed for the physician's evaluation and go to the designated Rural Health Unit with the complete requirements.
