Issuance of Death Certificate (Out-of-Facility Death, Medically Attended)
The City Health Office is responsible for reviewing death certificates. It examines the cause of death, issues the certificate, and directs the registration of the death at the City Civil Registry within the reglementary period of 30 days. The spouse or nearest relative of a person who died without medical attendance must report the deceased within 48 hours.

How to apply

Accomplish the death certificate form with the deceased's personal data and information.

Wait while staff record the deceased's details.
- 3
Provide the additional details needed, including the cause of death to be certified by the last attending physician and the embalmer's certification on the death certificate form.

Claim the completed and signed death certificate form.
