Issuance of Death Certificate (Medico-Legal in Nature)
The City Health Office is responsible for reviewing death certificates. It examines the cause of death, issues the certificate, and directs the registration of the death at the City Civil Registry within the reglementary period of 30 days. The spouse or nearest relative of a person who died without medical attendance must report the deceased within 48 hours.

Steps
How to apply

Accomplish the death certificate form with the deceased's personal data and information.

Wait while staff record and process the deceased's details.
- 3
Provide the details needed for the Post-Mortem Certificate of Death and follow staff instructions on completing the requirements.

Submit the deceased for autopsy at SOCO (Camp Olivas) for the Medico-Legal Officer's signature and the embalmer's certification on the death certificate form.
