Before You Apply
Requirements
Prepare these requirements before visiting the office.

Individual Treatment Record (1 original)
Where to secure: Barangay Health Station of the patient's present address

Gene Xpert or Sputum Microscopy result and/or Chest x-ray result (1 original, preferably with chest x-ray plate)
Where to secure: Any TB DOTS licensed/accredited health facility (for sputum); any laboratory and diagnostic facility (for x-ray)

Original Referral letter/form for TB enrollment/continuation of treatment/Prescription (if any)
Where to secure: Any private and public hospitals/clinics

Photocopy of any government-issued ID (preferably an ID with Name, Address, Birthday, Signature, and picture): Voter's I.D., National ID, Unified Multi-Purpose Identification (UMID), Social Security System I.D., Postal I.D., Tax Identification Number (TIN) I.D., Professional Regulation Commission (PRC) I.D., Driver's license, or Senior Citizen ID
Where to secure: Issuing agency (e.g., Commission on Elections, Philippine Statistics Authority, Government Service Insurance System, Philippine Postal Corporation, Bureau of Internal Revenue, Land Transportation Office, Office of the Senior Citizens Affair)
Steps
How to apply

Submit your Individual Treatment Record, laboratory/diagnostic results, referral letter or prescription (if any), and valid government ID.

Respond to questions during contact-tracing and health education regarding your treatment and HIV screening.
- 3
Listen to instructions on where to get your daily anti-tuberculosis medication, proper handling and storage, possible side effects, and your TB booklet and sputum follow-up schedule.
