Burial, Cremation, Transfer and Exhumation Permit
The City Health Office is responsible for the issuance of burial, cremation, transfer, and exhumation permits. This is a requirement for every entombment within the city, as the permit must be issued as required by law under P.D. 856 governing the disposal of the dead.

How to apply

Submit a copy of the reviewed/registered Death Certificate at the City Health Office.

Pay the required fee (PHP 200.00) at the City Treasurer's Office and wait for the official receipt.

Wait for the Burial, Cremation, Transfer, or Exhumation Permit to be prepared and issued.
