Requirements

Prepare these requirements before visiting the office.

    Immunization record of previous immunizations (if applicable)

    Where to secure: Hospital, Birthing facility, or other health facility where the mother gave birth or where the child received previous immunizations

    Parent(s) or Legal Guardian must accompany the child

Steps

How to apply

    The parent(s) or legal guardian presents a valid ID at the Rural Health Unit or Barangay Health Station.

  2. 2

    Give your consent for immunization and receive a queue number; the infant or child will be registered and issued an immunization card.

  3. 3

    Allow the health worker to check the vital signs and measurements of the infant or child, and receive information about the vaccine(s) to be administered.

  4. 4

    Allow the health worker to administer the vaccine(s), listen to instructions on possible side effects and after-care, and note the schedule for the next visit if applicable. If the infant or child is not well during the visit, the health worker will instead provide out-patient consultation and inform you of the next immunization schedule.
