The Central Pharmacy under the City Health Office serves the 35 barangays of the City of San Fernando, Pampanga, and provides free basic essential medicines to all Fernandinos.

Requirements
Prepare these requirements before visiting the office.

Kayabe Card (1 original)
Where to secure: Office of the Community Affairs, City Hall

Senior Citizen's Card (1 original), for patients 60 years old and above
Where to secure: Office for Senior Citizens Affairs (OSCA), Heroes Hall

Any government-issued ID showing City of San Fernando, Pampanga residence (1 original)
Where to secure: Issuing government office / Barangay Hall of residence

Barangay Certificate (1 original)

Voter's Certification or Voter's ID (1 original) — proof that the patient/client is a CSFP voter
Where to secure: COMELEC, Heroes Hall; or the Barangay Health Station of the patient's permanent residence

Where to secure: Rural Health Unit or private doctor
Steps
How to apply

Present your Kayabe Card, Senior Citizen's Card, or other applicable ID, and proof of residency (Barangay Certificate, Voter's ID, or Voter's Certification).

Present your latest prescription, signed by the attending physician.

Wait while staff check the authenticity of your prescription and the availability of the medicine(s).

Wait while staff record the details of your dispensed medicine(s).
- 5
If the requested medicine(s) are not available, staff will return your prescription or referral form to you.

Wait while staff prepare and double-check the medicine(s) to be given to you.
- 7
Receive your medicine(s), listen to the instructions on how to take them, and sign the End-User Form.
