Availment of Himlayang Fernandino Services
The City Health Office – Environmental Health and Sanitation Division (EHSD) is responsible for processing applications for Himlayang Fernandino niche and interment services.
Service at a Glance
Bona fide residents of the City of San Fernando, Pampanga (Fernandinos are exclusively vested to avail of the facility's burial services) who have resided in the city for at least six (6) months, as evidenced by a Barangay Certificate of Residency from the Barangay Chairman covering where the beneficiary resided during their lifetime.

Prepare these requirements before visiting the office.

Certificate of Indigency for the informant and for the deceased (1 photocopy each)
Where to secure: Barangay Hall of the client's permanent residence

Voter's ID or Voter's Certification of the informant and the deceased (1 photocopy each)
Where to secure: COMELEC, City Hall

Cedula (Community Tax Certificate) of the informant (1 photocopy)
Where to secure: City Treasurer's Office (Windows 1–4)

Assessment Report (medical/financial form) (1 original)

Steps
How to apply

Present all requirements at the City Health Office – EHSD to apply for leasing of a niche or interment.

Have your fee assessed based on the applicable lease/service type (see fee note), and wait while your application is processed.

Receive the Order of Payment and settle payment at the City Treasurer's Office.
- 4
Sign the Contract of Lease (for above-ground niche leases) and bring your copy of the Order of Payment to the City Mayor's Office as required.

At Himlayang Fernandino (Barangay Lara), present your Field Order Slip to the in-charge caretaker and bring the needed materials for the burial/interment.
