Before You Apply
Requirements
Prepare these requirements before visiting the office.

Court order requesting Drug Dependency Evaluation (DDE), duly signed by a judge and/or the court's authorized representative — or, for individuals who voluntarily self-surrender to undergo DDE, a request from the Barangay Council.
Where to secure: Trial Court (for court-ordered DDE) or Barangay (for self-surrender requests)

Original drug test result
Where to secure: DOH-accredited drug testing hospital or stand-alone laboratory
Steps
How to apply

Fill out the Drug Dependency Evaluation (DDE) request form and submit your court order and/or Barangay request, along with your original drug test result.

Undergo the initial assessment with the health care provider.

Wait for your scheduled DDE date. Note: scheduling depends on health care provider availability, generally within 24–72 hours, Monday to Thursday.

Proceed to the health facility on your scheduled date and undergo the Drug Dependency Evaluation, answering the health care provider's questions.
