Photocopy of any valid government-issued ID showing Name, Address, Birthday, Signature, and picture. For minors, the guardian or parent must present a photocopy of their own valid government-issued ID with the same details.

How to apply

Present your requirements and valid ID at the Dental Clinic; a staff member will register you and give you a queue number.

Consult with dental clinic staff; your vital signs will be taken and your medical and dental history assessed.

Undergo the restoration or pit-and-fissure sealant procedure; processing time may vary depending on the number of teeth treated. Receive home-care instructions afterward.

Sign the end-user/client feedback form before leaving.
