The San Fernando Super Health Center Central Laboratory, under the City Health Office, provides basic laboratory services to all Fernandinos to ensure quality laboratory results.

Requirements
Prepare these requirements before visiting the office.

Kayabe Card (1 original)
Where to secure: Office of the Community Affairs, City Hall

Senior Citizen's Card (1 original), for patients 60 years old and above
Where to secure: Office for Senior Citizens Affairs (OSCA), City Hall

Any government-issued ID showing City of San Fernando, Pampanga residence (1 original)
Where to secure: Issuing government office / Barangay Health Station (BHS) / Rural Health Unit

Laboratory Request Form (1 original)
Where to secure: Rural Health Unit of the patient's permanent area of residence; City Medical Unit for City employees
Steps
How to apply
- 1
Sign in at the Laboratory's general entry logbook.
- 2
Submit your laboratory request to the Central Laboratory for verification. Make sure to secure the Order of Payment that will be issued to you.
- 3
Pay the required fees at the City Treasury Office (Cashier's satellite office) by presenting the Order of Payment. Make sure to secure the Official Receipt issued upon payment.
- 4
Have your specimen collected, or submit your specimen, for the ordered laboratory test(s). Processing time varies by test.
- 5
Wait for and claim your laboratory results.

Important notes
- The total processing time of 1 hour, 30 minutes to 3 working days depends on which laboratory test(s) are ordered; most tests take about 4 hours, while a small number (e.g. TB testing) take up to 3 working days, per the Charter's per-test processing-time listing.
