Where to secure: Any laboratory and diagnostic facility

Photocopy of any government-issued ID (preferably an ID with Name, Address, Birthday, Signature, and picture): Voter's I.D., National ID, Unified Multi-Purpose Identification (UMID), Social Security System I.D., Postal I.D., Tax Identification Number (TIN) I.D., Professional Regulation Commission (PRC) I.D., Driver's license, or Passport
Where to secure: Issuing agency (e.g., Commission on Elections, Philippine Statistics Authority, Social Security System, Government Service Insurance System, Philippine Postal Corporation, Bureau of Internal Revenue, Professional Regulation Commission, Land Transportation Office, Department of Foreign Affairs)
Steps
How to apply

Present your identification and any prior chest x-ray result to the health worker for an initial assessment of TB signs and symptoms.

Receive the laboratory request form (Form 2A) and specimen bottle, and proceed to the Rural Health Unit as instructed.
- 3
Submit your sputum specimen(s) as instructed, following the schedule and number of specimens required, and wait to be contacted once the result is released.
