Steps
How to apply

Submit the completed Ambulance Request Form and the patient's current hospital records to the HEMS Office at Heroes Hall, CSFP.

Wait while HEMS personnel review the submitted documents and conduct a brief interview to verify the request.

Wait while HEMS staff assess the patient's condition and confirm the appointment with the receiving facility, subject to approval by the HEMS Medical Coordinator/City Health Officer.

Wait for a text message or call from HEMS personnel informing you whether the request was approved or disapproved, along with further details if approved.
