Prepare these requirements before visiting the office.

    Certificate of Live Birth (Municipal Form 102) (4 originals)

    Where to secure: CCRO or applicable birth attendant

  2. 2

    PSA Negative Omnibus Certification (1 original) and local no-record birth certification (1 photocopy)

    Where to secure: PSA and applicable LCRO

  3. 3

    Affidavit for Delayed Registration (4 originals) and barangay indigency certification (1 original)

    Where to secure: Applicant and Barangay

    Available proof of identity (1 photocopy)

    Submit the complete BRAP birth-registration documents at CCRO Window 47.

    Wait through the required posting and CCRO evaluation, registration, and approval.
