Prepare these requirements before visiting the office.

Applicable timely-birth documents and Certificate of Live Birth (Municipal Form 102)
Where to secure: CCRO and applicable birth attendant

Affidavit for Delayed Registration (4 originals), local no-record certification (1 photocopy), and PSA Negative Certification of Birth (1 photocopy) issued no more than six months before submission
Where to secure: Applicant, applicable LCRO, and PSA

Barangay certification, National ID, two unedited 2x2 photographs, registrant affidavit, and affidavit of two disinterested persons

Steps
How to apply

Submit the complete delayed-birth registration documents to CCRO.
- 2
Wait through the required 10-day posting and CCRO evaluation, encoding, registration, and approval.
- 3
Receive the registered local document.
Important
Important notes
- Under the PSA advisory dated May 4, 2026, the PSA Negative Certification of Birth must be no more than six months old.
- Out-of-town registration and PSA national availability are separate processes involving the applicable LCRO and PSA.
