Requirements
Prepare these requirements before visiting the office.

Certificate of Live Birth (Municipal Form 102) (4 originals)
Where to secure: CCRO or applicable birth attendant

Affidavit for Delayed Registration (4 originals)
Where to secure: Applicant

PSA Negative Certification of Birth (1 original), issued no more than six months before submission
Where to secure: Philippine Statistics Authority

Local certification of no birth record (1 photocopy)
Where to secure: Applicable LCRO
- 5
DSWD CNSP certification and medical or dental certification of approximate age (1 original each)
Where to secure: DSWD and applicable medical or dental professional
Steps
How to apply

Submit the complete CNSP birth-registration documents to CCRO.

Wait while CCRO evaluates, encodes, assigns the registry number, and obtains approval.
- 3
Receive the registered local document.
Important
Important notes
- Under the PSA advisory dated May 4, 2026, the PSA Negative Certification of Birth must be no more than six months old.
- CCRO local registration remains separate from PSA national record availability and certificate issuance.
