Prepare these requirements before visiting the office.

Registered Certificate of Live Birth (4 photocopies)

Steps
How to apply
- 1
Submit the complete RA 9255 legal-instrument documents to CCRO.
- 2
Pay PHP 200 at the City Treasurer's Office and return the official receipt.
- 3
Receive the locally registered legal instrument after processing and approval.
Important
Important notes
- RA 9255 permits use of the father's surname but does not itself change legitimacy status.
- CCRO local registration is separate from PSA national annotation and Security Paper issuance.
