Affidavit of Supplemental Report and two supporting documents; the Charter prints '1 photocopy' without clearly allocating the copy count
Where to secure: Applicant and applicable record holders

Applies only: Specified correction pathway
PSA document (4 photocopies), discrepancy or explanation affidavit (4 originals), and identification/cedula (1 photocopy each)

Steps
How to apply
- 1
Submit the requirements for the applicable component pathway to CCRO.
- 2
Pay PHP 200 at the City Treasurer's Office and return the official receipt.
- 3
Receive the locally registered instrument or transfer documentation after processing and approval.

- This remains one numbered Charter service with component pathways, not multiple services.
- The Charter's ambiguous 'two supporting documents / 1 photocopy' wording is preserved; applicants should confirm the required copy count with CCRO.
