Requirements
Prepare these requirements before visiting the office.

Registered Certificate of Live Birth (4 photocopies) and PSA marriage certificate
Where to secure: PSA or applicable LCRO

Identification and cedula of each affiant (1 photocopy each), plus CENOMAR or Advisory on Marriages

Steps
How to apply

Submit the complete legitimation documents to CCRO.
- 2
Pay PHP 200 at the City Treasurer's Office and return the official receipt.
- 3
Receive the locally registered legal instrument after processing and approval.

- Philippine judicial recognition is an external prerequisite where a foreign-divorce scenario applies.
- CCRO registers the instrument locally; PSA national annotation and certificate issuance remain separate.
