Applies only: For an out-of-city court or judicial recognition of a foreign judgment
Applicable registration certificate from the LCRO where the court functions or where a foreign judgment was registered (4 certified copies)

Applies only: When registered more than 15 days after the decision
Affidavit for delayed registration (1 original)

Steps
How to apply
- 1
Submit the final court documents and applicable civil-registry record to CCRO.
- 2
Pay PHP 500 at the City Treasurer's Office and return the official receipt.
- 3
Return after seven days for the registered decree documents.

Important notes
- The court supplies the judgment and Certificate of Finality; CCRO does not adjudicate the underlying case.
- PSA national annotation and certificate issuance are separate downstream processes.
