Requirements

Prepare these requirements before visiting the office.

    Court Decision and Certificate of Finality (4 certified copies each)

    Where to secure: Court

    Relevant Certificate of Live Birth, Marriage, or Death (1 photocopy)

    Where to secure: PSA or applicable LCRO

  3. 3

    Applies only: For an out-of-city court or judicial recognition of a foreign judgment

    Applicable registration certificate from the LCRO where the court functions or where a foreign judgment was registered (4 certified copies)

    Where to secure: Applicable LCRO

  4. 4

    Applies only: When registered more than 15 days after the decision

    Affidavit for delayed registration (1 original)

Steps

How to apply

    Submit the final court documents and applicable civil-registry record to CCRO.

  2. 2

    Pay PHP 500 at the City Treasurer's Office and return the official receipt.

  3. 3

    Return after seven days for the registered decree documents.

  • The court supplies the judgment and Certificate of Finality; CCRO does not adjudicate the underlying case.
  • PSA national annotation and certificate issuance are separate downstream processes.
