Requirements
Prepare these requirements before visiting the office.

Certificate of Finality, Order of Adoption, and draft new Certificate of Live Birth (4 certified copies each)

Existing PSA or local Certificate of Live Birth (4 photocopies)

Steps
How to apply
- 1
Submit the endorsement, final adoption documents, and existing birth record to CCRO.
- 2
Pay PHP 500 at the City Treasurer's Office and return the official receipt.
- 3
Return after seven days for the registered documents.
Important
Important notes
- NACC or RACCO adjudicates the adoption and issues the final order; CCRO registers the completed adoption.
- Any nationally available new Certificate of Live Birth is issued through PSA, not by CCRO as a PSA certificate.
