The City Civil Registry Office receives applications and issues City marriage licenses to qualified couples when either or both applicants reside in the City of San Fernando and no legal impediment exists.

Before You Apply
Requirements
Prepare these requirements before visiting the office.

Personal appearance of both applicants

Application for Marriage License forms (Municipal Form 90 / Form 2)
Where to secure: CCRO Window 47

Birth certificate, or baptismal certificate if the birth certificate is unavailable (2 photocopies)
Where to secure: PSA, CCRO, or the applicable church

Current Community Tax Certificate / cedula (2 photocopies), 2x2 photograph (1 each), and an accepted valid ID (2 photocopies)
Where to secure: Applicable issuing authority

Certificate of No Marriage Record (CENOMAR) or Advisory on Marriages (1 original and 1 photocopy)
Where to secure: Philippine Statistics Authority

Pre-Marriage Counseling or Orientation completion certificate (1 original)
Where to secure: City Health Office / Population Office

Applies only: As applicable to age, civil status, or citizenship
Applicable parental consent or advice forms, prior-marriage documents, legal-capacity certificate, and foreign-divorce documents

Steps
How to apply

Both applicants appear personally and submit the complete requirements at CCRO Window 47.

Receive the order of payment and pay the applicable fees at City Treasurer's Office Window 37.
- 3
Complete the separate City Health Office pre-marriage counseling or orientation prerequisite and submit its certificate.
- 4
Wait through the statutory 10-day posting period, then return for release and pay the PHP 2 marriage-license form fee.

Important notes
- Personal appearance and the statutory posting period are required.
- Pre-marriage counseling or orientation is a separate City Health prerequisite; its processing time is not added to CCRO's Charter total.
