Prepare these requirements before visiting the office.

    Barangay Certificate of Residency or Barangay Indigency of the client and beneficiary (1 original)

    Where to secure: Respective Barangay

    Applies only: For guardians

    Certificate or Affidavit of Guardianship (1 original)

    Where to secure: Respective Barangay or Legal Office

  3. 3

    School ID of the client (original for presentation and 1 photocopy)

    Where to secure: Requesting client

    Registration or Admission Confirmation or Certificate of Enrollment

    Where to secure: School where the client is enrolled

  5. 5

    Photocopy of Voter's ID, Voter's Certification, or two accepted valid IDs with specimen signatures of the parent or guardian

    Where to secure: Parent or guardian or issuing agencies

Steps

How to apply

    Visit the CSWDO, obtain a queue number, wait for your turn, and ask about the service requirements.

  2. 2

    Participate in the assessment interview, answer the questions, and wait for the decision.

  3. 3

    Upon approval, receive the Social Case Study and sign the logbook to acknowledge its release.
