Prepare these requirements before visiting the office.

    Barangay Certificate of Residency or Barangay Indigency of the client and beneficiary (1 original)

    Where to secure: Respective Barangay

    Death Certificate (original or certified true copy for presentation, with 1 photocopy of front and back)

    Where to secure: City Civil Registry Office

  3. 3

    Funeral Contract (original for presentation and 1 photocopy)

    Where to secure: Funeral service provider

  4. 4

    Applies only: Submit one accepted identification document

    Photocopy of Voter's ID, Voter's Certification, or another accepted valid ID with specimen signature of the client

    Where to secure: Requesting client or issuing agency

Steps

How to apply

    Visit the CSWDO, obtain a queue number, wait for your turn, and ask about the service requirements.

  2. 2

    Participate in the assessment interview, answer the questions, and wait for the decision.

  3. 3

    Upon approval, receive the General Intake Sheet and sign the logbook to acknowledge its release.
