How to apply

    Fill out the Veterinary Service Request Form and undergo an interview.

  2. 2

    Provide details of your request and sign the logbook.

  3. 3

    Allow the animal to be handled for the administration of the vaccine or treatment.

  4. 4

    For vaccination, sign the logbook and receive the Dog Card or Vaccination Record. For treatment, sign the logbook and receive the Animal Check-Up / Post-Treatment Management documentation.

  • Walk-in and on-site processing times are distinct; on-site requests take substantially longer than walk-in requests. Vaccine or medicine availability and staff scheduling are not guaranteed, and emergency or outbreak response may affect deployment. Dated barangay vaccination campaigns are not permanent availability.
  • Fees and processing times are attributed to the 2026 Citizen's Charter, 2nd Edition; they have not been separately reconfirmed by a newer source.
