Paano mag-apply

Register at the Rural Health Unit or Barangay Health Station for prenatal or postpartum care, and have your vital signs checked.

Seek consultation at the Rural Health Unit or Barangay Health Station.

Allow the health worker to conduct a physical examination, and receive the laboratory request form.

Bring the accomplished laboratory request form to the CSFP Central Laboratory for testing.

Listen attentively as the health worker orients you on proper maternal nutrition, maternal care, and possible danger signs and complications of pregnancy. Receive your Home-Based Mother's Record/Booklet ni Nanay, any medications provided, and instructions on your next pre- or post-natal visit, tetanus toxoid immunization schedule, medicine and supplement intake, and home-based aftercare.

If you are at least 7 months pregnant or considered a high-risk patient, receive a referral slip and instructions on continuing your care at a Birthing Station or your hospital of choice.
