Photocopy of any government-issued ID (preferably showing name, address, birthday, signature, and picture). Accepted IDs include Voter's ID, National ID, Unified Multi-Purpose ID (UMID), Postal ID, Tax Identification Number (TIN) ID, PRC ID, Driver's License, and Senior Citizen ID.

Submit your requirements and wait while staff review them for completeness and validity.

Allow the health worker to take your vital signs and conduct an initial physical examination before you are referred to the physician for signing of the medical certificate.

Wait for confirmation that your documents are available for claiming.

Claim the requested documents at the Rural Health Unit and sign the receiving logbook.

- A medical certificate for medical assistance is issued only if the medicines and laboratory work prescribed and requested are not available at the health center and central laboratory.
