A public health initiative specifically designed to register and provide treatment to individuals diagnosed with Drug-Resistance Tuberculosis to ensure they receive the necessary care, medication, and follow-up.

Individual Treatment Record (1 original)
Saan kukunin: Barangay Health Station of the patient's present address

Gene Xpert results and/or Chest x-ray result (1 original, preferably with chest x-ray plate)
Saan kukunin: Any TB DOTS licensed/accredited health facility (for sputum); any laboratory and diagnostic facility (for x-ray)

Original Referral letter/form for TB enrollment/continuation of treatment/Prescription (if any)
Saan kukunin: Any private and public hospitals/clinics

Photocopy of any government-issued ID (preferably an ID with Name, Address, Birthday, Signature, and picture): Voter's I.D., National ID, Unified Multi-Purpose Identification (UMID), Social Security System I.D., Postal I.D., Tax Identification Number (TIN) I.D., Professional Regulation Commission (PRC) I.D., Driver's license, or Senior Citizen ID
Saan kukunin: Issuing agency (e.g., Commission on Elections, Philippine Statistics Authority, Government Service Insurance System, Philippine Postal Corporation, Bureau of Internal Revenue, Land Transportation Office, Office of the Senior Citizens Affair)

Paano mag-apply

Submit your Gene Xpert results, Individual Treatment Record, laboratory/diagnostic results, referral letter or prescription (if any), and valid government ID.

Sign the treatment enrollment contract and respond to questions during history-taking and contact tracing.

Undergo the required physical and psychological examination, and complete any referred baseline tests (clinical evaluation, bacteriological, diagnostic, and blood tests) as instructed.

Listen to instructions on where to get your daily anti-tuberculosis medication, proper handling and storage, possible side effects, and your TB booklet, monitoring schedule, and sputum follow-up.
