Teresita, Juliana, San Jose; RHU II (Sindalan) – Baliti, Malino, Panipuan, Pulung Bulu, Sindalan, Telabastagan; RHU III (San Nicolas) – Del Pilar, Pandaras, San Felipe, San Pedro, San Juan, Sta. Lucia, San Nicolas; RHU IV (San Agustin) – Del Carmen, Maimpis, Magliman, Quebiawan, San Agustin; RHU V (Northville) – Bulaon, Calulut, Malpitic; RHU VI (Super Health Center) – San Isidro, Alasas, Del Rosario, Dela Paz Norte, Dela Paz Sur, Saguin, Lara.

Present your requirements and valid ID at the Dental Clinic; a staff member will register you and give you a queue number.

Consult with dental clinic staff; your vital signs will be taken and your medical and dental history assessed.

Undergo oral prophylaxis (cleaning) and, if needed, fluoride application. Receive home-care instructions afterward.

Sign the end-user/client feedback form before leaving.
